Liver Transplant

Liver Transplant

A liver transplant involves replacing a diseased liver with a healthy one from a donor, either a whole or partial liver. The liver can regenerate, so the donor's liver will grow back to normal size after surgery. This procedure addresses various indications for liver transplantation and involves specific types of donor livers and post-surgery immunosuppression (it refers to the use of medications to suppress or weaken the immune system’s activity) to ensure success.

What is a Liver Transplant?

A liver transplant is a surgical procedure to replace a diseased liver, often due to chronic liver failure like cirrhosis, with a healthy liver from a donor. The process involves various indications for transplantation, donor liver types, the surgery itself, and post-operative immunosuppression to prevent organ rejection.

Other conditions that may lead to end-stage liver disease or require a liver transplant include:

•    Acute hepatic necrosis

•    Biliary atresia

•    Viral hepatitis (Hepatitis B or C)

•    Alcoholic hepatitis

•    Nonalcoholic fatty liver disease (NAFLD) or nonalcoholic steatohepatitis (NASH)

•    Bile duct cancer

•    Metabolic diseases

•    Liver cancer (primary liver cancer)

•    Autoimmune hepatitis

Conditions Treated by Liver Transplant:

Liver Failure (Acute or Chronic): Severe liver damage requiring a transplant as a last-resort treatment.

Primary Liver Cancer (Hepatocellular Carcinoma): Transplant may be necessary if cancer is confined to the liver and the liver is in poor condition.

Acute Liver Failure: Sudden severe liver damage from toxins or infections, sometimes recoverable but can be life-threatening.

Chronic Liver Failure: Advanced liver disease, often cirrhosis, where significant liver tissue is replaced by scar tissue.

Primary Liver Cancer: Cancer originating in the liver; a transplant might be needed if cancer cannot be managed with partial liver resection.

Transplant Procedure:

Liver transplant surgery typically lasts six to twelve hours under general anesthesia. During the procedure, various tubes will be used to manage bodily functions:

•    IV Catheter: For fluids and medications, usually in patient’s arm or hand.

•    Additional IV: For blood pressure monitoring and blood samples, in patient’s neck or thigh.

•    Endotracheal Tube: Inserted into your windpipe, connected to a ventilator to assist with breathing.

•    Drainage Tubes: Placed in your abdomen to remove blood and fluid around the liver.

•    Nasogastric Tube: Inserted through your nose into the stomach to drain stomach secretions.

•    Bladder Catheter: For urine drainage.

Conditions Affecting Liver Transplant:

You may be ineligible for a liver transplant if you:

•    Have an untreated chronic infection

•    Suffer from metastatic cancer

•    Have severe heart or other major health issues

•    Have a serious non-liver condition that won’t improve post-transplant

•    Cannot adhere to a treatment plan

•    Engage in excessive alcohol consumption

Risks of a Liver Transplant

Some risks from liver surgery may include:

•    Bleeding

•    Infection

•    Blocked Blood Vessels

•    Bile Leakage or Blockage

•    Temporary Liver Dysfunction
